Many reasons: If you really have strep (culture positive), you may have a resistant bug that needs a stronger antibiotic. You could also have some other infection, like mono, which is not improved by antibiotics and may be better with steroids. It could be chronic reflux, and the antibiotics are making it worse. There could be a tumor which would need further assessment. If not improving, see your doctor.
