Medications; depends: Discuss this with your physician. Tophi are collections of uric acid. Medications that lower uric acid shrink tophi if the level of uric acid in blood can be lowered below 6mg/dl and the lower, the faster (but slowly; millimeters per month). If one has many bulky tophi and pills do not help over time, an infusion of pegloticase (krystexxa) can be much faster but is only rarely needed.
