Hard to tell: A stool sample could be analysed for evidence of tapeworm or other similar agent. Blood tests can also give some evidence. However, some people are thin because they have the genetics/metabolism for it.

Not necessarily: If you are having other symptoms, then possible, but would need some easy fecal tests to diagnose. I believe in another question you've lost weight. While this could be a sign of a parasite as well, it could be, since you felt like you should mention how many sweets you eat, possibly early diabetes type ii- frequent urination? Abdominal pain? If you can, run it by your primary care doc.
