Examine by Doctor: Often times worms, especially pinworms lay their larvae at night by the anus. So this causes the discomfort of itching by their movement. If you've had this for months, and it's been itching you may be re-infecting yourself through the larva being under your fingernails. So the best treatment is to go to your doctor, and have this examined closely to rule out hemorrhoids vs. Worms, and get it treated appropriately.

Before treatment...: Could you be referring to fissures, fistula, abscess? Sexually transmitted disease (syphilis, gc, hiv, etc.) should be considered. Hemorrhoids ; rectal prolapse can appear as local discomfort. Anal trauma of any kind may cause mechanical injury. Skin eruptions need to be ruled out ; addressed. Any possibility of pinworms? Get evaluated so the correct treatment can begin please.
