Sleepiness: Yes, excessive sleepiness can be a symptom of an underlying medical problem, such as sleep apnea, hypothyroidism, anemia, cardiac disease, diabetes. You should see your primary care physician to have an evaluation and possible some blood tests to narrow down the diagnosis.
