Can someone have fluid in estachian tube without having fluid in middle ear?

Not likely: It is virtually impossible to demonstrate fluid in the eustachian tube clinically or even radiologically. Eustachian tube obstruction leads to negative pressure in the middle ear and eventually fluid in the middle ear space. If the ear drum on inspection is considered to be normal, then it is highly unlikely to have eustachian tube obstruction.
