Musculoskeletal: Sounds like you need to see a medical professional as it isn't getting better, the head/neck/shoulder pain and hand swelling all might be related and need to be evaluated.

Sounds muscular: Without more detail on how long sx present, any h/o trauma, character and degree of pain it is hard to suggest a diagnosis. Could be a migraine, could be a positional muscle strain, could be neck arthritis with a pinched nerve, could be something more serious. If severe and going on more than a short time, get in to see your pcp.

Is it: Pounding or throbbing? Does light or sound make your headache worse ? Are you nauseated ? If so you have a migraine headache . 80% of migraines are associated with neck pain. If it is not a migraine then it could be a terrible headache from the spasm in your neck. For a migraine take 3 excedrinrs and lie in a cool dark room and sleep it off. For the other take Ibuprofen or aleve (naproxen) , ice or heat, md.
