Possibly: Sorry to hear that you fell on concrete. The only way to know if anything is broken is to see your doc. He/she can determine this through tests and then help you. Peace and good health.

Possible: You may have broken one of the facial bones you landed on. But even if you did, there is nothing to be done about that unless you have life threatening symptoms - difficulty breathing, seeing, cognitive changes, etc.

Trauma: Yes you could have suffered a fracture along with the trauma. You should see your doctor or er for care.

Possible: It is possible you have an orbital fracture from your fall. If you have double vision, change in vision or severe eye swelling and pain you should see your eye doctor. They will get a ct scan of the orbits to rule out fracture if suspected on exam. Certain orbital fractures are treated while others are observed, however you should get it checked out if symptomatic.
