Open surgery: Pvns is a notoriously difficult problem to address. The disease behaves more like a tumor than inflammation. If arthroscopy has failed then open surgery might be required. A skilled fellowship trained orthopedic sports medicine specialist should be able to manage this. If you want a good academic center either campbell clinic or vanderbilt are good bets.
