Several options: For that kind of TMJ issue, usually an ENT (ear, nose, throat) doctor, dentist, or oral surgeon may treat you appropriately.

A cranial dentist: Sometimes having dental procedures of any kind done can lock up the TMJ joint. So the best person to start with is your dentist. S/he can recommend my other devices, or treatments (cranial) that could decompress the TMJ and neck, or use devices that would prevent you from grinding your teeth at night too that could exacerbate this, like your night guards. Then, you can see an osteopathic dr. To treat the rest of you.

TMJ/TMD specialist: Tmj/tmd is never cured, just maintained. It can come back without warning. I would see a tmj/tmd specialist who hopefully can educate you what do do when flare ups occur.
