However perthes @ age 4 generally does not cause as signifigant problems as those who develop it later in adolescence; so hopefully u are not developing arthritis ... Best option is to see an orthopedic surgeon for evaluation and x-rays . Best of luck!

Young for arthritis: Perthes is a pediatric hip condition that not only causes hip pain at the time, but can predispose to other hip issues, such as arthritis. Nowadays, there are other options for treatment of hip disorders before there is significant arthritis. If you haven't seen a hip preservation surgeon (someone who could offer you something other than hip replacement), i would definitely do that. Good luck!

No: Your age is not too young to develop arthritis if you had erthe's at age 4. It depends on how you were treated then and what the outcome was. You should see a hip and knee reconstruction specialistp.
