Exam is appropriate: You have localized your problem to the peri-anal area. The problem may be GI related (e.g. Colonic/ rectal inflammation, mass, rectal/anal dysfunction, colonic dysmotility), parasites but may be extrinsic to the GI tract (dermatologic, bladder, and in women peri-uterine, vaginal). Exam is definitely warranted for diagnosis ; appropriate treatment.
