Nothing by itself: It may be a lab error. Or it may be a tipoff that you have actual subtle disease. If the history and physical exam aren't helpful, then forget about it. The sed rate (ESR) is a poor tool to use for a fishing expedition, but it's especially helpful in picking up those musculoskeletal conditions that may be inflammatory (lupus, polymyalgia rheumatica, others).
