Surgically if painfu: If your thrombosed external hemorrhoid is painful, you should see a surgeon about having it "lanced" for instant relief. This can usually be done in the office or er under local anesthesia and should totally resolve the issue for that hemorrhoid. However, there are usually several hemorrhoids (commonly 3 columns) so a recurrence in a different hemorrhoid is not uncommon.
