Fertility and tumors: There are three primary reasons for bringing the testes from an undescended position to the dependent scrotum. The scrotal position has a 1 to 2 c lower temperature environment which is conducive for improved fertility potential and decreasing malignant testicular tumor formation. The scrotal testes is able to be palpated for self-examination. The third reason would be for cosmetics.

Undescended testes: ... Are associated with: 1. Increased risk of infertility. 2. Increased risk of testicular cancer. 3. Increased risk of testicular torsion (twisting) or trauma. Those are therefore the indications for surgery to bring the testis down into the scrotum (orchiopexy). A 4th reason of lesser importance would be how the genital situation looks.

The following: 1) Cryptorchid testis will have impaired or absent sperm production. Testis requires slightly reduced environmental temperature of scrotum 4 sperm production. 2) Slight increase risk of testicular cancer, so testis should be brought down so can be diagnosed early & cured. 3) Psychological. Most self respecting males want 2 testes down. 4) Slightly greater risk of testis torsion or trauma.
