It should not matter: A person who is brain dead is actually dead. That means he already has passed away, and the "time of death" on his death certificate should show the time that the person became brain dead. The person is not alive just because machines are there. No such letter should be needed, but it may help to have one if a person lives in where politicians or courts have tried to keep dead people on machines.
