Depends: If large uterus (20+ weeks, but might be earlier), back sleep can reduce blood flow/return. If don't like true side sleeping, can use a body pillow or wedge under right side (left side down) to keep about 30-45 degree angle with some benefit. Talk to your ob, look online for carrying-twins websites / blogs, as not a new problem ; different options exist.
