Yes, is possible: Fat grafting of the lower extremities is difficult due to the edema and blood supply but can be performed. It may take multiple sessions and will require a donor site.

Fat graft for calf: Calf augmentation is most commonly done with implants. I prefer to do it with fat grafting because it is a more natural look and has less complications. Additionally, if an implant is needed later on the increased subcutaneous tissue depth masks the appearance of the implant for a more natural looking result. It does require a period of bed rest and leg elevation.

The procedure that I have perfected provides permanent fat take with results that are either near or the same as the normal leg.
