Internet Medicine: I am sure you can understand but there really are limits to internet medicine. Tears in the perineum are in no way usual, normal or common. To have symptoms for over a year makes this even more unusual. I honestly think there is no way to help you with out a direct examination and perhaps even testing as to why your skin is not healing. Please get to the family clinic soon for an examination.
