Could be from reflux: Testicular veins do not get inflamed as result of varicoceles. However pain ; inflamation of epididymis, causing swollen tender scrotum cab be caused by reflux of urine into ejaculatory ducts ; from there into vas deferens. Epididymitis requires antibiotics, non-steroidal antiflamatory meds like Ibuprofen or acetominophen ; rest. An ice pack may help, ; a urologist certainly can. Good luck.
