Thyroid changes: From what you say, it appears something has changed from your first visit to the subsequent ones. 25 mcg of thyroxine should not make you hyperthyroid unless your thyroid is also making substantial t4/t3. You don't mention symptoms. There are a number of different forms of thyroiditis, or autoimmune thyroid disease, that can cause changes over time. You should perhaps see an endocrinologist.

Elevated TSH with nl: When ft4 is normal and TSH elevation may indicate subclinical hypothyroidism or acute illness like surgery and pregnancy or any condition that increases thyroid binding hormones. In those cases thyroid replacement are not indicated and may lead to hyperthyroidism which could show suppressed TSH in 8 to 12 weeks.
