Could an ultrasound detect appendicitus?

Yes: If ultrasound shows an abnormally enlarged appendix, it's almost always appendicitis. However, the appendix can be hard to find with ultrasound, especially in large patients. An enlarged appendix is easier to find than a normal one. The skill of the person scanning and the time spent looking impact the ability to find the appendix. Ct is more sensitive but involves radiation.
