It could be a sign : Of a problem but does not have to be. Some kids are idiopathic toe walkers. Are there any other signs of developmental delay? Try putting heel lifts in the shoes, but would recommend a neurology consult. (pediatric neurologist).

Doubtful: Persistent tip-toeing could be from tight heel cord and can also be a part of cerebral palsy.

Criteria is more.: Social give and take problems: not having good eye contact to not being able to join up with peers ideas for play. Prefer to be alone/not seeking to start social interaction. This is central and it is not situational e.g like a social anxiety. Two other key areas: social communication probs and restricted patterns of behavior.
