At the age of four the child still walks on his tip-toes ... i know that's not normal could it be a sign of autism..

It could be a sign : Of a problem but does not have to be. Some kids are idiopathic toe walkers. Are there any other signs of developmental delay? Try putting heel lifts in the shoes, but would recommend a neurology consult. (pediatric neurologist).

Doubtful: Persistent tip-toeing could be from tight heel cord and can also be a part of cerebral palsy.
