Creatine won't impact it, or cause telogen effluvium or alopecia areata. You can get a prescription from your physician, consider hair transplantation, wear a ballcap or hairpiece, or embrace your genetic heritage like photo guy.

Recently, a clinical study of the brazilian soccer team showed an increase in Dht (dihydrotestosterone), the primary hormone trigger for hereditary hair loss, after taking creatine supplements.
