Fertilization point: Text books and references usually count time from fertilization. Most people won't even think about a pregnancy until one period is missed though.

No way to know: As you say, the convention of doctors is to measure pregnancy from the start of the last periods- I have never understood this as it means you are 2 weeks pregnant at the moment of conception. But since there are thousands of books ; websites it is impossible to know if they all abide by the same convention. But if they are discussing time from conception i would hope they would say so.
