No: An insurance policy cannot force you to have a particular therapy. However, the doctors in these plans are evaluated on how well they follow guidelines for great patient care, such as do they vaccinate their patients. So this encourages doctors to encourage you to get appropriate vaccines. It's just good medicine and the plans are trying to get you the best health care, including preventive care!

Nope: Although vaccinations are an effective public health and disease control tool, they are not absolutely required. Most requirements for schools etc. will have religious or philosophical exemptions. Employers may take a different opinion and require for example flu vaccination for healthcare workers but include options like taking time off work, wearing a mask etc instead of vaccinations.
