Acromio-clavicular: It's where your clavicle (collar bone) connects to your shoulder blade. Check out http://en.Wikipedia.Org/wiki/acromioclavicular_joint.

Acromioclavicular : Its the joint where the clavicle joins the acromion. Its the tip of the shoulder and is commonly injured when you fall on shoulder. The joint capsule is stretched and the clavicle lifts to varying degrees depending on the amount of trauma.
