Rectal exam: Blood in stool is a sequelae of a rectal problem, either bleeding hemmorhoids or a bleeding growth or rectal fissure. Either could cause bleeding with or without pain. Discharge from the rectal would suggest some form of inflammatory process, but if discharge is from vagina in the case of a female patient, the two would be unrelated.
