Lots of reasons...: First off, you didn't mention if heart or lung testing was accomplished, and if so what were the results. These organ systems must be addressed. Assuming these were "normal", then functional issues must be considered like: esophageal spasm, esophageal dysmotility, anxiety/stressors, reflux, or it may be referred pain. Get checked to be certain a potentially critical issue is not ignored.
