No one needs to know: When you come in for surgery, you put on a hospital gown. No one needs to see the lower left side of your back. The anesthesiologist may want to listen to your lungs, but you don't have to uncover much. Wear cotton underwear and it should be ok to leave it on. If you are embarrassed about the tattoo, consider having it removed by laser.
