Decreased blood flow: For older patients peripheral vascular disease is the most common cause. Other factors that can decrease blood flow to the toes would be smoking and exposure to cold. In younger patients, especially women, raynaud's disease can cause blue toes when there is exposure to cold temperatures.

Many possibilities: Some causes of blue toes include: impaired venous or arterial circulation, cold exposure, trauma, and raynaud's.
