Different issues: Falling ntprobnp (or just bnp) indicates improved compensation but rising ef indicates true improvement in cardiac condition. For instance, someone with an ef of 10% may receive medication and ntprobnp will fall from 10, 000 to 5, 000 as they feel better. But someone with a viral cardiomyopathy may get no treatment but, in time, their ef may rise from 30% to 50%. Exercise tolerance poorly predicts.
