Is it true that an enlarged spleen is always associated with other diseases?

No: Some folks have a biggish spleen for no reason. If disease is present, and if often is especially if the spleen is very large, a good history and physical exam with a CBC and chemical profile will detect it. At autopsy, i've found spleens up to 1000 gm for no obvious reason.
