Cardiac vs. visceral: The most serious thing could be cardiac problems causing pain radiating to the arm and chest. Also, ulcers and pancreatic problems can irritate the diaphragm and cause similar symptoms. It could be something simple like anintercostal spasm, but this does not usually go to the arm. Chestwall abnormalities are also a possibility. See your family doctor for evaluation and testing.
