Get MRI: This is your opinion or your pcp opinion?I would see an orthopedic surgeon w/ arthroscopy fellowship training or foot/ ankle fellowship training.( or podiatrist). A history , physical exam and x-rays would be performed... If this has already been done get a good- quality MRI scan and a talar dome fracture should be easily seen if present. Treatment if present can then begin. Gl!
