No, by no means!: You may have a very creative little girl - enjoy her. This is likely to be a curious child trying to figure things out - she may have noticed that the same person uses the formal or intimate names at different times and she's trying to understand the implications (in terms of goodies) - how perceptive she is! worry can do nothing good and it might deprive you, her and us all of a lot of joy.
