More precise: Lateral internal sphincterotomies treat chronic anal fissures. Traditionally the internal sphincter muscle was divided all the way up to the dentate line. This sometimes resulted in incontinence. Tailored sphincerotomy divides the muscle only as high as the fissure goes. Some surgeons modify this if they measure for anal stenosis. Studies show less risk of incontinence with tailored procedures.
