Two choices: This is either an anteriorly placed fat pad or a dermoid. The former is benign although it might be unsightly and could be removed at some later date. Dermoids are local growths of skin related tissues including fat which can grow, get inflamed and sometimes interfere with function. Have a pediatric ophthalmologist look at your 4 month old.
