Many things: May be plantar fasciitis? May be a nerve related issue? Check with your regular doctor or podiatrist. If plantar fasciitis, stretching exericises, orthotics, and the opinion of a podiatrists are all helpful. If not plantar fasciitis, then other tests need to be performed.

Bone spur: You may have a thickening of the heel bone in the back of the heel. This is where the achilles tendon attaches. The tendon itself can also be inflamed. You need to check shoes that may be rubbing against the back of the heel, ice.
