Not usually: Spermatoceles can cause mild scrotal discomfort from the "mass effect" of having a collection of water in the scrotum. If you walk differently because of the weight or mass effect of a large spermatocele (say the size of a soccer ball) then, over time, theoretically, you could develop hip issues. I have never seen it.

No, ...: No. It's just impossible to cause hip misalignment because of its far distance from hip. The largest spermatocele that I have ever operated upon for its inconvenient bulkiness was some 15 cm in its longest dimension as big as a hydrocele, so resulting in confusion even with ultrasound study, but confirmed during its surgery.
