Need to assess : Possible sources of back pain which may be related to structural issues of spine in terms of facet joints or discs leading to mechanical instability - in some cases with wide decompressions leading to stress fractures of remaining bone ie. Pars region. Need spine surgeon to assess with standing flexion/extension films, ct scan ; contrast MRI if failed pt, etc unless smoker then high failure rate.

?scar tissue: Sometimes scar tissue postoperatively can cause the same symptoms that were initially treated. You need to return to see your physician to reassess why you are having so much pain.
