Could be various : Things. It could be slower healing, or also could be recurrent pressure from a disc, or residual stenosis still present, and still putting some pressure on nerve. Also at time with moving nerves gently to get all the pressure off, nerves get excited/inflammed for a few weeks and later calm down -- nerves @times don't like to be touched, but gentle retraction is needed @times during surgery.

Could be one of: Several issues. It could be some scar tissue forming which usually occurs at this time period that becomes chronic in 1-2% of patients usually with a several week period initially post op of no pain. It could also be a recurrent herniation as well or retained fragment if persisted from surgery or it could just be nerve root inflammation. If persists by 12 weeks, get MRI with contrast to check.
