Many things: Could be a thrombosed hemorrhoid, abscess, or tumor. Have a colorectal surgeon examine you and make the diagnosis and offer the appropriate treatment/managment.

Another hemorrhoid: Probably another hemorrhoid that has thrombosed. This means it has a clot in it. If it goes away within a couple weeks, no concerns. If it doesn't get it checked.
