Can an old case of active tb continue without killing the person and undetected for more then a year just causing persistent cough and breathlessness?

Yes: Tb is very slow - it can simmer for a long time with slowly progressive symptoms. And diagnosis is difficult because the organism is hard to catch and cultures are very slow to grow. So, yes, it's not uncommon to make a diagnosis after it's been present for a long time.
