Multiple reasons: Painful menstrual cramping can be a sign of many different things like infection, an anatomic problems like fibroids, or possibly endometriosis. It could just be a normal but accentuated response of uterine wall muscle to the inflammatory chemicals of menses. See your gynecologist to be evaluated properly.
