Nerve, occlusion: Large decay tends to have post op reaction if the cavity is too close or already invade into the nerve. In that situation, your dentist sometimes used direct pulp cap in avoidance of the root canal therapy and the added cost to his or her patient (good dentist!) sometimes the nerve reacted and inflamed. This require root canal. Check occlusion since the high filling also cause acute pain.
