Yes: It could be any/all of those. Also, gastroesophageal reflux disease, peptic ulcer disease, non-ulcer dyspepsia, esophageal spasm, fibromyalgia, pleurisy, costrochondritis, cervical spondylitis as well as a host of heart problems (but not heart failure). Please see your doctor!

Quality of pain: The answer is the type or quality of pain being felt. The location, severity, onset, precipitating factors etc all can help medical personnel figure out what's going on. Sudden crushing chest pain radiating to the jaw or down an arm is easily distinguishable from a sore rib that hurts when pushed.
