Depends: Depending on who is actually performing and reading the ultrasound will determine your actual risk. A highly experienced maternal fetal medicine doctor will have a very low risk of missing a baby with down syndrome.

1 of many variables: Approximately 50% of babies with Down syndrome have some sonographically identifiable abnormality. Whatever your risk is based on either your age or the adjusted risk based on testing (such as the ultrascreen or tetra/quad screen), you can cut that risk in half if there is a "normal" targeted ultrasound.
