Depends on opiate: Most meds are out of your system after 5 half-lives. Methodone has a 24 hour half life, so you may block any residual up to 5 days later. Most others are shorter acting. As a rule of thumb, if you are not having withdrawal symptoms and are several days past last use, i would say you are safe to start naltrexone.
