What would happen if a kidney stone was too big to pass but it went untreated?

Depends: A large kidney stone that does not pass may obstruct the flow of urine from the kidney to the bladder, causing the "urine collecting system" of the kidney to dilate or expand. If this happens for a prolonged period, damage to the kidney may result, decreasing its overall function. Stones more than 5 mm may need treatment and should be addressed by a urologist.
